I'm no doc, but intuitively - The sleeves from the Drs squeeze and relax because the people they are meant for are basically bed or chair-ridden. They get no leg exercise at all so the sleeve has to do the work.

A runner does the squeeze/relax every time their foot hits the ground and comes up again. I personally would not have a sleeve that was too tight at the beginning of a run because my calf muscles will expand from exertion, make the sleeve tighter, and possible reduce circulation.
