Hi Docs,

I've been wearing minimalist shoes for running for nearly two years. About a year ago I started to get a numb 2nd toe on my right foot when running. The numbness starts to occur at about the 6-8km distance and steadily gets more uncomfortable until about 10-12km then I don't really notice it too much. The area of numbness is on the left side of the tow next to the big toe near the end - pretty much from the last knuckle up. I've found splaying my toes as I'm running gives temporary relief after a sharp bit of pain. I also found that scrunching my toes about 10 - 15 times every km or so tends to alleviate the problem a little, or more delays the onset.

The other thing if I've found the problem is worse when I'm wearing my vibram fivefingers than when I'm wearing my xero shoes. I still get it in the Xeros but because I can move my toes around more it seems to help. I'm at a bit of a loss about this as I have normally resolved all of the other injury problems but this one just seems to keep hanging around. I've done the prodding and squeezing to check for Morton's neuroma and there is nothing there between the metatarsal heads. My second toe is about 1mm longer than my big toe and the metatarsal head of the second toe sits just ahead of the big toe metatarsal head so I guess there might be some thing in that. The left foot has no problem though and it's the same shape as the right one.

Any ideas? I'd certainly appreciate any advice.
 
My guess is that the problem is likely localized. Not sure if you have considered either a mortons toe syndrome (numbness in the toe as you describe) is typical, a possible problem in the posterior compartment in the calf on that side (muscle or tendon scar tissue and restriction of movement of the flexor hallicus or possibly the other muscles that control the digits. There may be problems in your core as well, causing you to slam the forefoot of that foot into the ground.

Just some ideas, but not sure unless I evaluate you in the office. If you would like, post a video of you running from the side on this forum which can help me help you better.

Thanks for the reply, the Morton's toe looks like it might be the problem. Anyone got any ideas how to stick a pad to the underneath of the 1st metatarsal head without it falling off (the pad, not the metatarsal head)?

We have a thread called Fellowship of the Morton's Toe here: Fellowship of the Morton's Toe It's quite long, but there is some talk about toe orthotics for MT there.
 
Thanks for the link, I tried small pad under my 1st metatarsal head yesterday but I think it was too small, a slight difference maybe. But after reading the morton's toe thread I'm going to try something with my invisible shoes to see if that works. It's a very good thread with loads of helpful information.

What about this? Take a wide elastic band, measure it to wrap around your big toe and your second toe, then sew it so it is taught, not tight, sort of like a spint. Or am I off-base here...docs?

Also, when you are running...and walking...concentrate on toeing-off from your big toe and not your second toe. Just be conscious about it. See if you can make a difference that way.
 
Thanks for the advice. I did what you recommended about concentrating on working my big toe. I kept making sure my 1st metatarsal head was doing the work. What also helped was the large piece of rubber I stuck to my invisible shoes about 2mm thick, with about 1mm of double sided tape (the thick stuff for hanging pictures). Well, this actually works! No 2nd toe problem on the 10km run this morning - though there's a new blister point, oh well can't have everything! I'll keep with this configuration for a couple more runs to see if things settle down. Anyway, right now, I'm very happy. Thanks for the advice, thanks for a great forum! This problem has been bugging me for ages so it's great to finally make progress. I was limited to about 12-14km with this toe thing now I reckon I can probably get back to the half marathon distance, and maybe more..... :)
